Serious Case Reviews
Regulation 5 of the Local Safeguarding Children Boards Regulations 2006 requires LSCBs to undertake reviews of serious cases.
The prime purpose of a Serious Case Review (SCR) is for agencies and individuals to learn lessons to improve the way in which they work both individually and collectively to safeguard and promote the welfare of children.
In all cases the SCR overview report should be used to produce an executive summary that should be made public and which accurately reflects the full overview report. The executive summary is anonomysed but will include information about the review process, key issues arising from the case, the recommendations and the action plan (including any actions that have been completed).

Serious Case reviews carried out by the DSCB in the last 12 months
There have been no Serious Case Reviews (SCR) over the last 12 months carried out by the DSCB and information on all archived cases are stored by the DSCB office.
